Coordinated Reentry

Across the country, more than two million adults and juveniles are incarcerated in nearly 7,000 facilities, including state prisons, federal prisons, local jails, juvenile correctional facilities, immigration detention facilities, Native American Country jails, military prisons, civil commitment centers, and state psychiatric hospitals. Chronic illness, addictions, and co-morbidities are often experienced by people in the carceral system. Coordinating a person’s reentry into a community of providers is proven to be effective in getting people on the right track and makes a difference in the number of re-offenses and recidivism rates. The complexities increase when a person is unhoused, suffers from mental illness and drug addiction, and is living with chronic health conditions. The reentry process starts prior to release from the facility and branches into the community’s network of caregivers, health providers, and resources.

Serrano’s approach to collaborated reentry begins with the custody operations teams and engages the reentry partners through problem-solving, data exchange, workflow development, and operational readiness. A landscape analysis is completed to inventory the community resources and to serve as a guidepost for the reentry planning. Technology and automation are combined with existing practices to simplify the reentry procedures and to maximize revenues for the pre-release services.

Serrano Advisors published a series of free toolkits to support reentry partners.

Strategic Planning

In a market where volatility is certain and change is constant, how do you position your organization to sustain in Medicare and Medicaid? The federal cuts to Medicaid spending exceed $880 billion over the next ten years and California represents over 12% of the national expenditures. Governor Newsom held the first press release on May 14 to outline the budget cuts and the Medicaid expansion population is highlighted for change on January 1, 2026. More changes are proposed to the Medi-Cal program to control spending and to reduce the budget deficit. Building and executing an adaptive strategy requires thinking outside the box. Whether you are considering a strategic expansion, corporate turnaround, merger, or sustainability, an actionable plan is necessary to communicate to your Board of Directors. Strategies are anchored as the cornerstones into the strategic plans, and often become accented by goal setting, accountability, follow through, and execution. Serrano offers a unique blend of facilitation services to create actionable strategies and to harness the strengths of your organization at every level.

Stakeholder Engagement

Implementing the justice involved reentry program is reinforced through interagency collaborations. The County Board of Supervisors, Office of the CEO, Office of the Sheriff, Probation Department, Social Services, Public Health, Behavioral Health, and managed care health plans are searching for ways to work together. Each agency plays a critical role in the reentry process and the managed care health plans have been introduced as the newest reentry partner. Engaging with stakeholders for the development and continuity of a successful reentry program is reinforced by governance. Serrano leverages the fundamentals of engagement to gather reentry partners and to focus on multi-agency collaboration.
